1. A procedure is for one eye.  The award is good for one Custom IntraLase™ or Standard LASIK procedure and must be performed within one year of the drawing date.
2. Recipient must be at least 18 years of age. One registration per person allowed.
3. Recipient must register online, by phone, or in person.
4. Recipient must undergo a no charge, no obligation LASIK eye evaluation at Piedmont Eye Center.
5. Recipient must be a qualified candidate as determined by the evaluation. Piedmont Eye Center will not perform procedures on anyone whose evaluation indicates that the procedure may not be medically beneficial.
6. Award is non-transferable.
7. Award neither carries nor conveys a cash value.
8. Random drawings of qualified candidates will be conducted by Piedmont Eye Center on April 30, August 29, December 31, 2008.
9. If award recipient already had procedure at Piedmont Eye Center between January 1, 2008 and December 31, 2008, a refund will be given for one procedure.
10. Limited time offer, promotional giveaway ends December 31, 2008.
11. Award recipient agrees to participate in any promotional campaigns for Piedmont Eye Center, if requested.
