1. A procedure is for one eye.  A bi-lateral procedure is for two eyes.
2. The award is good for a bilateral Custom IntraLase™ or Conventional IntraLase™ procedure and must be performed within one year of the drawing date.
3. Recipient must be at least 18 years of age. One registration per person allowed.
4. Recipient must register online, by phone, or in person.
5. Recipient must undergo a no charge, no obligation LASIK eye evaluation at Piedmont Eye Center.
6. Recipient must be a qualified candidate as determined by the evaluation conducted by Darin K Bowers, MD. Piedmont Eye Center will not perform procedures on anyone whose evaluation indicates that the procedure may not be medically beneficial.
7. Award is non-transferable.
8. Award neither carries nor conveys a cash value.
9. Random drawings of qualified candidates will be conducted by Piedmont Eye Center on April 23 and December 31, 2010.
10. If award recipient already had a LASIK procedure at Piedmont Eye Center between January 1, 2010 and December 31, 2010, a refund will be given for the procedure performed.

12. Award recipient agrees to participate in any promotional campaigns for Piedmont Eye Center, if requested.
